Benefits Include:

1. 401(k) Plan :

 

Description :

 

Pretax and post-tax Roth savings through payroll deduction
Company match of employee contribution: 25 cents to every dollar up to 4% of base salary
Employee may enroll, suspend and make changes to contributions at anytime

 

Eligibility :

 

All regular status employees age 18 and older
Qualifying plan dates following 6 months of hire:
Jan 1
Apr 1
Jul 1
Oct 1

 

Employee Contribution :

 

Minimum of  $1
Maximum of 100% of salary up to annual IRS deferral limit, and less other
elective deferrals and required deductions
Make up contributions permitted for employees age 50 and over

2. Holidays :

 

Description :

 

10 company designated holidays per calendar year

On declared calendar days and Floating Holidays

 

Eligibility :

 

All regular FT employees Excluding Band 1 Hourly employees
Must be on active pay status day before and day after holiday

 

Employee Contribution :

 

None

3. Vacation :

 

Description :

 

All Full time employees in Band 2, 3, 4, 5 and Band 1 Salaried: 2 weeks accrual per year

After 5 years of employment: 3 weeks accrual per year

 

Eligibility :

 

All regular FT employees

Prorated from Date of Hire

 

Employee Contribution :

 

None

4. PTO :

 

Description :

 

Band 1 Hourly employees – 10 days accrual per year (pro-rated as per the start date)

After 6 months of service (as on Jan 1st of the following year) – 12 days accrual per year.

After 18 months of service (as on Jan 1st of the following year)  – 15 days per year.

 

Eligibility :

 

All regular FT employees Prorated eligibility in the year employee is hired - from Date of Hire to Dec. 31st

 

Employee Contribution :

 

None.

5. Sick Leave :

 

Description :

 

Used for personal illness/doctor’s appointments

 

Eligibility :

 

All Full Time employees in Band 2, 3, 4, 5 and Band 1 Salaried

Prorated from Date of Hire

 

Employee Contribution :

 

None

6. Tuition Assistance :

 

Description :

 

Employees may seek assistance for tuition and fees for academic education.

Reimbursement of up to $1500 per calendar year for cost of tuition, books, and parking for courses toward continuing education, certification program, associate's degree or a bachelor's degree

 

Eligibility :

 

Regular FT employees in all bands

None

7. Medical Plan :

 

Eligibility :

 

All regular full time employees
Spouse or same sex domestic partner and dependent child(ren) of eligible employee
Date:
Band 1 & 2 – 1st of the month following 60 days of employment

Band 3, 4, 5 –1st of the month following 30 days of employment

 

Employee Contribution :

 

Cost Sharing depending on type of plan and type of coverage chosen by the employee.

 

Description :

 

5 different plan options offered through Aetna, including HDHP with HSA account. Company contributes pre-tax money to employee HSA account.

8. Dental Plan :

9. Vision Plan :

10. Dependent Care & Health Care Reimbursement Accounts :

 

Eligibility :

 

All regular full time employees.

Must enroll during open enrollment period in December.

 

Description :

 

Enables employees to pay for dependent care and health expenses with pretax dollars

 

Employee Contribution :

 

Maximum $4,000 until 12/31/2012 and $2,500 starting 1/1/2013.
$5000 for Dependent Care

11. Health Reimbursement Account :

 

Eligibility :

 

Only those who elect HDHP plan may participate in the HSA.

 

Description :

 

HSAs combine the benefits of both traditional and Roth 401(k)s and IRAs for medical expenses. Taxpayers receive a 100% income tax deduction on annual contributions, they may withdraw HSA funds tax-free to reimburse themselves for qualified medical expenses, and they may defer taking such reimbursements indefinitely without penalties.

 

Employee Contribution :

 

Individual may contribute up to $3,250 and family $6,450.

In addition QuEST global will contribute $250 for individual, $500 for employee and spouse, $400 for Employee and children and $1000 for family.
